How to Create Fillable Forms in Adobe

Bon Talampas | Aug 20, 2010 | 12 comments

Adobe Acrobat, is capable of creating fillable PDF forms for applications, surveys or questionnaires. You can insert a variety of fillable form fields into your PDF document: Text fields allow users to type text into a designated text box, check boxes are used to select an answer (such as “Yes” or “No”), and list boxes provide a predetermined set of responses from which users may choose. The steps that follow are for Adobe Acrobat 9, the latest version of the software.

1. Open your existing PDF document.  ( I use registration form sample)

2. Go to “Forms” and then select “Add or Edit Fields…”

3.  A popup window may appear asking if you would like Adobe to detect form fields. Choose “No.”

4.  Click on “Add a New Field” and select Text Field.

5. Once selected, move your mouse to the location in the document where you would like to place the fillable field.

6. Click and drag the mouse to customize the size of the field. Type in the name of the fillable field. Please note: Users will not see this title. It is only visible in the form editing mode

7.  To edit Text Field properties (such as font size, font color, border); right click the Text Field and then select Properties.

Inside Text Field Properties

8.  You can click preview to test and see the result if the text field is working

9. You can return to “edit page” and then click “File” menu then select  Save or Save as, if you want to save as a new file.

Done. =)

    hey, thanks for the tips. but as the title says, this is if you are going to create the editable pdf file.

    question, pano kung may mga editable pdf files pero sinasabi niyang kapag niclose mo siya, di ka pwedeng magcreate ng saved file with the filled out form? walang magagawa talaga sa ganun? i am talking about the forms you download from the net which are in pdf format. marami sa kanila, pwedeng ifill out while on the pc and then you just print it when you are done. pero kung di mo natapos ang pagfill out ng form, di mo na rin siya pwdeng i-save and any data will be lost. :-( wala na nga bang magagawa sa mga ganung pdf forms?
